You can claim Gift Aid if:

• You're a UK taxpayer.

• You pay enough UK Income Tax and/or Capital Gains Tax in the current tax year to cover the amount of Gift Aid claimed on all your donations.*

• You are not paying in donations made by a third party, e.g. money collected at an event, the pub, a company donation or a donation from a friend or family member.

• This donation is not made as part of a sweepstake, raffle or lottery and I am not receiving anything in return of it, e.g. book, auction prize, ticket to an event.

If you're unsure whether you qualify, you can still donate without Gift Aid.

*If you pay less Income Tax and/or Capital Gains Tax than the amount of Gift Aid claimed on all your donations in a tax year, it's your responsibility to pay the difference to HMRC.

By completing the declaration, you’re confirming that:

• you want us to claim Gift Aid on your eligible donations;

• you are a UK taxpayer; and

• you have paid (or will pay) enough UK Income Tax and/or Capital Gains Tax to cover the Gift Aid claimed on all your donations.

Your declaration allows Plymouth Hospitals Charity to claim Gift Aid on eligible donations you make in the future and donations you have made to us in the previous four years.

Please let us know if you change your mind or if your circumstances change and you no longer pay enough UK Income Tax and/or Capital Gains Tax to cover the Gift Aid claimed on your donations.

You should also tell us if you:

• change your name or home address;

• want to cancel your Gift Aid declaration; or

• no longer want us to claim Gift Aid on your donations.
